This Service Agreement (“Agreement”) is entered into between Cypress Wellness Solutions LLC ("CWS") and the undersigned client ("Client") for the provision of Pulsed Electromagnetic Field (PEMF) therapy services to human, equine, small animal, and livestock recipients.
CWS provides non-medical PEMF wellness services using certified MagnaWave equipment. Sessions are available for:
-Human Clients
-Equine, small animal, and livestock clients
-Full-body or targeted PEMF therapy
-Combo sessions for horse and rider
-Customized wellness or performance packages
Service descriptions and pricing are available on the CWS Rate Sheet and are subject to change with notice.
-Packages: Full payment is due at or before the first scheduled session.
-Individual Sessions: Payment is due at the time of service.
-Accepted Payment Methods: Cash, check, credit/debit card, and approved mobile payment apps.
-CWS provides mobile sessions within a 20-mile radius of Woodbine, MD.
-Travel beyond this radius is subject to a mileage fee of $1.00 per mile (round-trip).
-Mileage is calculated via Google Maps or a comparable service from the business address.
-Mileage fees may be waived or adjusted for group bookings or at the discretion of CWS.
-Clients must provide at least 24 hours’ notice to cancel or reschedule.
-Late cancellations (under 24 hours) may incur a fee equal to 50% of the session cost.
-No-shows will be billed at 100% of the scheduled session cost. Fees may be collected at the next appointment or invoiced.
Clients agree to:
-Disclose any relevant medical, behavioral, or physical concerns prior to each session
-Ensure animals are haltered, clean, and under control during equine, small animal or livestock sessions
-Follow all practitioner instructions for safe and effective treatment
-Understand that CWS reserves the right to decline or discontinue services if safety, behavior, or environmental factors pose a risk
Do you consent to Cypress Wellness Solutions LLC taking and using photos or videos of your session (or your animal’s session) for marketing, social media, or educational purposes?
